Add Time Worked field to Incidents

What
Request from Olaf/Mats to:
- extend the new Time Worked field added on Requests to Incidents as well,
- make it visible all the time (not only when Report Type is Proactive).
How
I created:
- a new field called 'u_time_worked' of type glide_duration in the incident table.
I left:
- the existing 'u_time_worked' field in the u_request_fulfillment table.
- the Proactive choice in the Report Type choice list (as requested by Olaf).
I deactivated (but didn't delete):
- the UI Policy on Requests so that 'Time Worked' always appears, and not only when Report Type is Proactive.
I put the 'Time worked' field just above Report Type for incidents and requests, as Report Type is common to both.
Warning:
- This field is ALWAYS VISIBLE (even at creation time).
- Changes to it don't appear in the Activity log, but they appear in the History (right click on bar).
